The forms that you will need depend upon whether you and/or your pets have been to University Hills Animal hospital before. If you are new to University Hills Animal Hospital, please complete the
New Client Information Form. For each new pet, please complete one New Patient Information Form. There are different forms for cats, dogs, and exotic pets.

If you do not plan to be with your pet during their examination, please complete the Patient Drop-Off Form. If you have any questions or difficulties, our receptionists or assistants will be happy to help you complete these forms.

Please print and fill out the forms that apply to your pet and bring them with you when you come in or you may fax them to us at (303.692.8925) them to us the day before your visit.The more complete and accurate the information on your documents, the better we can serve you and your pet.
